Who May Be Admitted?

  • Adults who require an assisted living level of care
  • Individuals who need help with daily living activities such as bathing, dressing, grooming, mobility, or medication management
  • Residents whose care needs can be safely met in an assisted living setting
  • Individuals willing to participate in an individualized care plan

Required Admission Documents

  • Completed admission application
  • Comprehensive medical evaluation from a licensed healthcare provider
  • Current medication list with dosage and instructions
  • Tuberculosis screening or other required health screenings
  • Physician’s orders, when applicable
  • Proof of health insurance, Medicare, or Medicaid information, if applicable
  • Government-issued photo identification
  • Emergency contact information
  • Advance directive or power of attorney documents, if available

Pre-Admission Assessment

Before admission, our care team conducts an assessment to review the resident’s physical, cognitive, emotional, and healthcare needs. This helps us determine the appropriate level of care and create a personalized care plan in collaboration with the resident, family, and healthcare providers.

What to Bring on Move-In Day

  • Comfortable clothing and non-slip footwear
  • Personal toiletries and hygiene items
  • Prescription medications in original labeled containers
  • Eyeglasses, hearing aids, dentures, walkers, canes, or other assistive devices
  • Favorite photos, decorations, and personal keepsakes
  • Important medical and legal documents, if requested
